HUD Manager.  Full-time in Lakeland.  This position maintains current and orderly tenant rental files, collects and documents tenant rent and security deposits in an orderly, professional and timely manner, and attends all necessary trainings to maintain Occupancy Specialist certification and to remain current on HUD rules and regulations. Forensic Residential Stability Coordinator Full-time in Bartow. This position is responsible for monitoring a small caseload of adult individuals with mental illness involved with the criminal justice system with the goal of helping to reach self-sustainability, developing a housing plan to work towards stable long-term housing and ultimately look to increase overall stability to support diversions from the State Hospital, readmissions to inpatient units or frequent arrests. This position requires regular travel, court appearances, and submission of status reports to the courts.       Behavioral Helath Therapist Targeted Case Manager Full-time in Avon Park. This position entails intensive school based therapeutic/clinical interventions for school age children up to age 17. The therapist must identify problem areas and formulate and implement an individualized treatment plan with the client and family. Clinical interventions also include supportive family counseling as needed as well as school based group counseling to facilitate peer support.       Home To Stay Program Manager Full-time in Lakeland.  The program manager is responsible for the overall supervision and daily operation of the Home to Stay program. The program is designed to assist with successfully reunifying children with their parents in Circuit 10 after removal due to abuse, neglect or abandonment by providing intensive in-home services as well as providing appropriate therapeutic and case management support to assist with the prevention of potential placement breakdown. The program manager supervises a team of Home to Stay Care Managers and Prevention Care Managers who work closely with the parents and the child/children in their homes and in the community to assist where needed in order to facilitate a successful reunification with their child and prevent further removal. Families are typically open with the Home to Stay program four to six months.                   CAT Team 2 Mentor. Full-time and Part-time in Lakeland. A CAT Mentor provides mentoring and support for children and adolescents receiving mental health services through the Community Action Treatment (CAT) Team. They assist families in navigating the mental health services system and in achieving resiliency and recovery as defined by the child and family. Mentors provide support services that are designed for the restoration, modification, and maintenance of social, personal adjustment, and basic living skills. These services are designed to assure that the recipient possesses the physical, emotional, and intellectual skills to live, learn, and work in their own environment. Respite services are provided when deemed necessary by the CAT Team. This position will be based in Polk County.                   CAT Team 2 Therapist Full-time in Lakeland. This member of the multidisciplinary team has training in one of the mental health professions (social work, rehabilitation counseling, or psychology) and clinical skills and experience in assessing, planning, developing, coordinating, and providing treatment, rehabilitation, and support services to program participants and families with severe and persistent mental illnesses under the clinical supervision of the CAT Team Program Manager and the Psychiatrist/ARNP.                   Charge Nurse Lakeland and Bartow. Full-time. The Charge Nurse is responsible for managing, supervising and assisting assigned staff as well as providing administrative support and direct patient care to patients and assistance to the medical staff by management of available resources in accordance with established standards, policies and procedures and the scope of practice established by the State of Florida. The Charge Nurse provides on-going real-time decision making and leads the inpatient interdisciplinary team in assuring a safe, therapeutic environment which supports clients in the stabilization of crises.                   Children's Targeted Case Manager, Lakeland, Full Time M-F 8-5 (flexible). $17.93 hourly. This is a Bachelor’s level position that requires the person to have the ability to prioritize workload and work independently. Assist recipients in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. Minimum of 1 years’ experience working with children with mental health related issues. This position is primarily covers services in Polk County. However services are available in Highlands and Hardee Counties as needs arise. Must be 21 years old with clean driving record.                   Crisis Outreach Counselor Full-time. The Crisis Outreach Counselor provides mental health crisis services in various settings for immediate crisis assessment, crisis intervention, and time-limited, crisis counseling to promote crisis resolution and evaluation to individuals in Polk County who have come to the attention of Polk County Sheriff’s Office and may be experiencing any type of emotional distress. Working in partnership with Sheriff’s Deputies, the Crisis Outreach Counselor facilitates continuity of treatment for individuals through follow up contact to assist with connection to services, establish plans to alleviate further crises, and facilitate the Baker Act process as needed. This position is more of a focus on answering crisis line, monitoring text/chat and Walk-in crisis assessments. Bachelor’s Degree in a Human Services field from an Accredited College or University.                   Forensic Multidisciplinary Team Case Manager. Full-time Bartow. $17.93/hr. This position is responsible for providing case management services within the Forensic Multidisciplinary team to a small caseload of adult individuals with mental illness who have been deemed incompetent to proceed with trial or Not Guilty by Reason of Insanity by the court. The Case Manager will monitor, link, assess, and plan with the overall goal of diverting the individual from placement in a forensic state hospital. The position requires regular travel, court appearances, and submission of status reports to the courts. The services provided include, but are not limited to, housing education, budget assistance, landlord mediation, social rehabilitation, employment assistance, SSI/SSDI Outreach Access and Recovery (SOAR), transportation, activities of daily living education, medication education, Public Housing Authority Voucher assistance, and service connection. This position provides in-person and telephonic counseling and advocacy, hospital accompaniment, information and referral connections, support groups accompaniment to court, and law enforcement interviews. The position requires the ability to be on call two weeks per month. A.A. Degree preferred. High school diploma/ G.E.D. At least two years working with sexual assault victims, women’s issues and/or other mental health issues. Relevant degree may be substituted for experience. The individual must have the competence to provide clinical assessments, individual, group, and family therapy. Position requires the therapist have knowledge of mental health and substance abuse diagnoses and treatment per the DSM-5. Requires a Master's degree in the human services field (psychology, sociology, social work, counselor education, etc.), abide by a code of ethical and practice standards, and be committed to a process of lifetime knowledge and skills development. Have a minimum of two years of direct experience working with individuals with a substance use diagnosis. The Clubhouse is a rehabilitation program for people with mental illness where real life job skills are taught and used to work towards recovery through meaningful activities to give the member the skillset to be a contributing member of society. This position would be a direct care position working with members to help them learn responsibility, develop self-esteem and learn to interact professionally with others. Hours are 8:00 a.m. to 5:00 p.m. Monday to Friday, some holidays and weekends. Must be 25 years of age with a valid driver’s license and clean driving record.
