Behavioral Health Navigator

POSITION TITLE: Behavioral Health Navigator

REPORTS TO: Project Supervisor

JOB STATUS: Full Time 

FLSA: Non Exempt

Job Summary: The Behavioral Health Navigator assists individuals in accessing the service delivery system for behavioral health care and wrap around services. The Behavioral Health Navigation Project is time-limited with a heavy concentration on educating and empowering the person served to engage in the appropriate level of treatment or care as well as community-based services, using natural supports in addition to system based services to facilitate connection. Services may be provided in the office or within the community at large.

Essential Duties/Requirements/Activities:

  • Facilitate linkage to identified services and serve as a point of contact, advocate, and informational resource for the individual.
  • Conduct follow-up contacts and re-evaluate crisis plan as necessary.
  • Complete initial assessments to determine client level of need.
  • Conduct crisis assessments/interventions when needed.
  • Develop individualized action plan with individuals which may include connection to additional services based on the individual's needs.
  • Maintain program documentation for statistical and outcome purposes as necessary.
  • Concisely and correctly complete clinical documentation and related paperwork in a timely and professional manner.
  • Ability to access resources and information and to translate this into clear action steps for clients to follow to access needed care.
  • Collaborative view of engagement with clients and willingness to come alongside an individual who is just starting their behavioral health care journey in order to access the appropriate level of care.
  • Good written and verbal communication skills as well as excellent customer service ability
  • Good interpersonal skills with peers and other agencies/organizations
  • Good organizational skills and ability to handle multiple tasks
  • Creative management and knowledge of community resources
  • Ability to keep information confidential
  • Ability to work with minimal supervision
  • Ability to travel to multiple sites
  • Provide timely, accurate, completion of required documents

Qualifications:

Education:

Bachelor's Degree in a Human Services field from an Accredited College or University

Experience:

Minimum of one (1) year of experience in mental health or related field.

Knowledge and Skills:

Assessment

Excellent written and verbal communication skills

Highly organized with the ability to work with minimal supervision

Excellent skills and knowledge of Microsoft Office

Standard Precautions

Comply with Occupational Safety and Health Administration (OSHA) rules and regulations

Life Safety Equipment (fire extinguisher)

Transportation:

Minimum 21 years of age

Drive personal vehicle and have own reliable transportation

Ability to pass Florida DMV license check

Must maintain 50/100 insurance requirements for Peace River Center designated drivers

Work Environment:

The work environment is fast-paced and consists of exposure to physical conditions typical of a normal office environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. Most of the job is performed while sitting, although the work may require occasional standing or walking and/or the lifting and carrying of small objects. A small amount of travel may be required for meetings etc. to another PRC location.
