Job Summary:

This position is responsible for providing case management services within the Forensic Multidisciplinary team to a small caseload of adult individuals with mental illness who have been deemed incompetent to proceed with trial or Not Guilty by Reason of Insanity by the court. The Case Manager will monitor, link, assess, and plan with the overall goal of diverting the individual from placement in a forensic state hospital. The position requires regular travel, court appearances, and submission of status reports to the courts. The services provided include, but are not limited to, housing education, budget assistance, landlord mediation, social rehabilitation, employment assistance, SSI/SSDI Outreach Access and Recovery (SOAR), transportation, activities of daily living education, medication education, Public Housing Authority Voucher assistance, and service connection.

Essential Duties/Requirements/Activities:

  • Regular travel to local jails, hospitals, clients' homes or assisted living facilities.
  • Link participants to needed services
  • Monitor service delivery to clients in the community and jails.
  • Accurate completion of all necessary documentation in a timely manner.
  • Assess client needs and provide community resource coordination.
  • Make frequent appearances in court.
  • Submit status reports to the courts in a timely manner.
  • Provide competency restoration training to individuals adjudicated incompetent to proceed.
  • Calaborate with other FMT members
  • Assist with discharge planning.
  • Work closely with supports such as family, local providers and aftercare services.

Qualifications:

Education:

Bachelor's Degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or related human services field or Certified Recovery Peer Specialist.

Experience:

Requires a minimum of one (1) year related field experience. (3) years preferred.

Certifications:

Certified Recovery Peer Specialist accepted in lieu of Bachelors in human services field.

Training:

Attend all PRC software, clinical and administrative educational functions as deemed appropriate by supervisor. Complete all trainings as required by Peace River Center.

Knowledge and Skills:

Requires good verbal and written communication.

Requires flexibility and the ability to prioritize job duties.

Must be capable of working independently with minimal supervision and as a member of a team.

Ability to effectively interact with court personnel.

Must be able to identify symptoms of mental illness.

Ability to effectively interact with individuals with mental illness and intervene in crisis situations.

Requires working knowledge of Microsoft Office - Outlook, Word and Excel.

Transportation:

Must have reliable transportation.

Requires clean driving record. DMV check will be performed on offer of employment.

Must have current Florida Driver's License and continue to maintain during employment.

Maintain current $25,000/$50,000 bodily insurance coverage on personal vehicle.

Must meet DOT driving requirements. Must be a minimum of 25 years of age to be a qualified driver.

Computer, telephone, fax, copier.

Work Environment:

The work environment is fast-paced and consists of exposure to physical conditions typical of a normal office environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. Most of the job is performed while sitting, although the work may require occasional standing or walking and/or the lifting and carrying of small objects. A small amount of travel may be required for meetings etc. to another PRC location.

A person in this position will work both in an office and at various sites including recipients' home, jail, and courthouse.
