Children's Targeted Case Manager Full-time in Lakeland.  This is a Bachelor's level position that requires the person to have the ability to prioritize workload and work independently. Assist recipients in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. A majority of duties will be in the community working with children and families providing services in-home, at daycare, in the school system, and in residential treatment facilities. This position will be based in Bartow and will primarily cover services in Polk County. DV Primary Prevention Advocate/DV OR Advocate FT/Lakeland This is a non-degreed position. The advocate is responsible for Primary Prevention activities as outlined in the DCF Office of Domestic Violence Contract. The Primary Prevention Advocate shall provide information, referral and counseling to survivors of domestic and sexual violence and the public. Provide presentations and training within the community and to co-workers. Facilitate support groups with required documentation attached. Act as a liaison with community partners including law enforcement and judicial system.     Forensic Multidisciplinary Team Case Manager. Full-time Bartow. This position is responsible for providing case management services within the Forensic Multidisciplinary team to a small caseload of adult individuals with mental illness who have been deemed incompetent to proceed with trial or Not Guilty by Reason of Insanity by the court. The Case Manager will monitor, link, assess, and plan with the overall goal of diverting the individual from placement in a forensic state hospital. The position requires regular travel, court appearances, and submission of status reports to the courts. The services provided include, but are not limited to, housing education, budget assistance, landlord mediation, social rehabilitation, employment assistance, SSI/SSDI Outreach Access and Recovery (SOAR), transportation, activities of daily living education, medication education, Public Housing Authority Voucher assistance, and service connection. Responsible for coordination of services, supports, and cross system collaboration for child recipients identified as High Need/High Utilizer (HNHU). The Care Coordinator facilitates continuity of treatment for recipients while in psychiatric treatment and assists the recipients in accessing the service delivery system subsequent to their discharge. Care Coordination is time-limited with a heavy concentration on educating and empowering the person served.     Adult Targeted Case Manager full-time in Lakeland. Responsible for assessing, linking, planning, monitoring and advocating for recipients who have serious mental illnesses to provide for their care while living in the community. Facilitates continuity of treatment for Peace River Centers catchment area recipient’s while at the in psychiatric, substance abuse and medical inpatient treatment and/or a state treatment facility and assist the recipients in accessing the service delivery system subsequent to their discharge. The target population served are persons who have been discharged from an inpatient setting, persons who are in need of linking and monitoring to prevent inpatient treatment, persons who lack the resources or ability to provide for their needs without assistance, and persons who have been arrested within the recent past.     Behavioral Helath Therapist Targeted Case Manager Full-time in Avon Park. This position entails intensive school based therapeutic/clinical interventions for school age children up to age 17. The therapist must identify problem areas and formulate and implement an individualized treatment plan with the client and family. Clinical interventions also include supportive family counseling as needed as well as school based group counseling to facilitate peer support. The DSM is to be utilized to determine the mental health diagnosis of the child and therapeutic/clinical interventions needed. This position also provides targeted case management support designed for the restoration; modification; and maintenance of social, personal adjustment, and basic living skills.     Children's Therapist Part-time/Full-time in Lakeland.  The therapist will be responsible for providing therapy and assessment services for both children and adults, while focusing specifically on the family, child, and adolescent populations. This position requires the individual to have a master’s degree in a mental health field. Preference will be given to individuals who are registered MHC, MFT, or MSW interns and actively pursuing licensure or have received their LMHC, LMFT, or LCSW credentials.         Clubhouse Generalist. Full Time. The Clubhouse is a rehabilitation program for people with mental illness where real life job skills are taught and used to work towards recovery through meaningful activities to give the member the skillset to be a contributing member of society. This position would be a direct care position working with members to help them learn responsibility, develop self-esteem and learn to interact professionally with others. Hours are 8:00 a.m. to 5:00 p.m. Monday to Friday, some holidays and weekends. Must be 25 years of age with a valid driver’s license and clean driving record.         Crisis Outreach Counselor Full-time. The Crisis Outreach Counselor provides mental health crisis services in various settings for immediate crisis assessment, crisis intervention, and time-limited, crisis counseling to promote crisis resolution and evaluation to individuals in Polk County who have come to the attention of Polk County Sheriff’s Office and may be experiencing any type of emotional distress. Working in partnership with Sheriff’s Deputies, the Crisis Outreach Counselor facilitates continuity of treatment for individuals through follow up contact to assist with connection to services, establish plans to alleviate further crises, and facilitate the Baker Act process as needed. This position is more of a focus on answering crisis line, monitoring text/chat and Walk-in crisis assessments. Bachelor’s Degree in a Human Services field from an Accredited College or University.         Director of Club Success in Lakeland.  Full-time. The Clubhouse Director is a leadership position that will report to the Director of Recovery Service of Peace River Center and to an Advisory Board and will have a strategic and operational responsibility for the Clubhouses in accordance with federal, state and local requirements. This position will expand and develop a deep knowledge of the community behavioral health field along with the Clubhouse International Standards while leading a team to ensure the running of successful Clubhouse programs.  Bachelors’ Degree in Human Services Discipline from an accredited College or University and 1 year experience in a Psychiatric Rehabilitation Program. Experience with program development, fund raising and community resource building preferred. Master’s Degree, LCSW, or LMHC preferred. Previous supervisory experience strongly preferred.         FIT Clinician Full-time in Bartow. This position is a position for a Master’s level therapist that is responsible for providing assessments and counseling services in the FITT (Family Intensive Treatment Team) Program. The individual must have the competence to provide clinical assessments, individual, group, and family therapy. Position requires the therapist have knowledge of mental health and substance abuse diagnoses and treatment per the DSM-5. Requires a Master's degree in the human services field (psychology, sociology, social work, counselor education, etc.), abide by a code of ethical and practice standards, and be committed to a process of lifetime knowledge and skills development. Have a minimum of two years of direct experience working with individuals with a substance use diagnosis. Experience working in an agency providing substance use services preferred.         This position will provide primary care services to adults and children who may have co-occurring behavioral health disorders in our primary care clinic and our Primary Care practice. Will work collaboratively with the Program Director to provide health and wellness education and care management.           Outreach Specialist RRP - Polk / FT / Lakeland This position provides services to primary & secondary victims of sexual assault age 12 and above. Provides telephone (hotline crisis calls and office calls), hospital call-outs (support during a forensic exam, sharing of information and referrals, assist participants with filing for Victim Crime Compensation claims) accompaniment to court, and Law Enforcement interviews. Provides ongoing counseling helping victim’s recover from their trauma. Facilitates and/or participates in providing support groups as well as attending community events to share information on awareness and prevention of sexual assault. Uses websites and local colleges to recruit for volunteers, provides training and supervision of volunteers. Facilitates monthly volunteer in-service meetings, and completes the minutes. Maintains volunteer files. Responsible for maintaining participant’s records and inputting information into the computer data systems. The ability to be on-call two weeks per month. Any other duties assigned by the Supervisor for business necessity. A.A. Degree preferred. High school diploma/ G.E.D. Graduation from an accredited medical school. Completion of an accredited Psychiatric Residency Training program. Licensed to practice medicine in the state of Florida. Assist with facilitating support groups within the Shelter and the community. Complete intake paperwork and service documentation for survivors of domestic violence who enter the Shelter. Comply with all nutritional Grant program requirements that include ensuring food is available per menu, food is set out at appropriate times; accurate documentation is completed immediately. Provides direct care to clients in the Crisis Stabilization Unit (CSU) and Short-Term Residential Treatment Unit (SRT). The CSU/SRT Inpatient Technician functions as a member of an interdisciplinary team to provide a safe, therapeutic environment and support clients in the stabilization of crises and reintegration into the community. This Lead Technician functions as an active part of the interdisciplinary treatment team, providing continuous client care, monitoring, supervision, interaction, and role modeling to adult and adolescent clients. The Lead Technician works to support the overall goal of stabilization of client crises to reintegrate into the community.
