Children's Targeted Case Manager Full-time in Lakeland. This is a Bachelor's level position that requires the person to have the ability to prioritize workload and work independently. Assist recipients in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. The therapist must identify problem areas and formulate and implement an individualized treatment plan with the client and family.
