Children's Targeted Case Manager Full-time in Lakeland.  This is a Bachelor's level position that requires the person to have the ability to prioritize workload and work independently. Assist recipients in gaining access to needed medical, social, educational, and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. A majority of duties will be in the community working with children and families providing services in-home, at daycare, in the school system, and in residential treatment facilities. CPI/Outreach Advocate/ FT/ Lakeland This is a non-degreed position that will require the advocate to work collaboratively in a consulting role with CPI child welfare staff, facilitate or co-facilitate DV training with/for DCF staff, safety plan with participants, explain DV services available, provide service management and referrals. The advocate will complete registration paperwork and service documentation for victims of domestic violence who enter Outreach or Shelter services and provide information, referral and counseling to survivors of domestic and sexual violence and the general public and facilitate support groups with required documentation attached.             Director of Club Success in Lakeland.  Full-time. The Clubhouse Director is a leadership position that will report to the Director of Recovery Service of Peace River Center and to an Advisory Board and will have a strategic and operational responsibility for the Clubhouses in accordance with federal, state and local requirements.               CAT Team 2 Program Manager Full-time in Lakeland.  The Program Manager establishes, administers, and directs the Children’s Community Action Treatment (CAT) Team 2, a self-contained, intergrated, multidisciplinary team providing comphrensive and intensive community based treatment to families with children and youth at risk of out-of-home placement due to a mental health disorder. The Program Manager supervises and evaluates the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program participants. Facilitates continuity of treatment for Peace River Centers catchment area recipient’s while at the in psychiatric, substance abuse and medical inpatient treatment and/or a state treatment facility and assist the recipients in accessing the service delivery system subsequent to their discharge. The target population served are persons who have been discharged from an inpatient setting, persons who are in need of linking and monitoring to prevent inpatient treatment, persons who lack the resources or ability to provide for their needs without assistance, and persons who have been arrested within the recent past.                       Behavioral Helath Therapist Targeted Case Manager Full-time in Avon Park. This position entails intensive school based therapeutic/clinical interventions for school age children up to age 17. The therapist must identify problem areas and formulate and implement an individualized treatment plan with the client and family. Clinical interventions also include supportive family counseling as needed as well as school based group counseling to facilitate peer support. The DSM is to be utilized to determine the mental health diagnosis of the child and therapeutic/clinical interventions needed. This position also provides targeted case management support designed for the restoration; modification; and maintenance of social, personal adjustment, and basic living skills.                       Charge Nurse Lakeland and Bartow. Full-time. The Charge Nurse is responsible for managing, supervising and assisting assigned staff as well as providing administrative support and direct patient care to patients and assistance to the medical staff by management of available resources in accordance with established standards, policies and procedures and the scope of practice established by the State of Florida. The Charge Nurse provides on-going real-time decision making and leads the inpatient interdisciplinary team in assuring a safe, therapeutic environment which supports clients in the stabilization of crises. This position reports directly to the CSU Operations Manager for administrative issues and to the Nurse Supervisor for medical/clinical matters.                       Clubhouse Generalist. Full Time. The Clubhouse is a rehabilitation program for people with mental illness where real life job skills are taught and used to work towards recovery through meaningful activities to give the member the skillset to be a contributing member of society. This position would be a direct care position working with members to help them learn responsibility, develop self-esteem and learn to interact professionally with others.                       Crisis Outreach Counselor Full-time. The Crisis Outreach Counselor provides mental health crisis services in various settings for immediate crisis assessment, crisis intervention, and time-limited, crisis counseling to promote crisis resolution and evaluation to individuals in Polk County who have come to the attention of Polk County Sheriff’s Office and may be experiencing any type of emotional distress. Working in partnership with Sheriff’s Deputies, the Crisis Outreach Counselor facilitates continuity of treatment for individuals through follow up contact to assist with connection to services, establish plans to alleviate further crises, and facilitate the Baker Act process as needed. This position is more of a focus on answering crisis line, monitoring text/chat and Walk-in crisis assessments. Bachelor’s Degree in a Human Services field from an Accredited College or University. This position is a position for a Master’s level therapist that is responsible for providing assessments and counseling services in the FITT (Family Intensive Treatment Team) Program. Responsible for the provision of direct care to the patients of the CSU in Lakeland or Bartow or SRT and provides assistance to medical staff by management of available resources in accordance with the established standards, policies and procedures at the Peace River Center’s Crisis Inpatient Facilities, and the Scope of Practice established by the State of Florida.                       Victim Services Licensed Therapist. Full-time in Lakeland. The therapist will be responsible for providing individual and group therapy and assessment services for both adolescents and adults, while focusing specifically on the resolution of traumatic experiences such as domestic violence and sexual assault. This position requires the individual to have a master’s degree in a mental health field and to have obtained their state licensure as either a LMHC, LMFT, or LCSW. The DSM is to be utilized to determine the mental health diagnosis of the child and therapeutic/clinical interventions needed. Master's degree in mental health field required. Reporting to the Peace River Center Medical Director, this position will provide and oversee direct care to adult and child/adolescent patients inpatient or outpatient services utilizing both office based face to face and a tele-health portal platform to provide psychiatric services. Graduation from an accredited medical school. Completion of an accredited Psychiatric Residency Training program. Licensed to practice medicine in the state of Florida. Current DEA number with no restrictions on prescribing controlled substances. Eligible for Medicare, Medicaid and other third party reimbursement. Individuals excluded from participation in federal reimbursement programs (including Medicaid, Medicare, and CHAMPUS) by the Office of the Inspector General do not meet the requirements for this position. These services are designed to assure that the recipient possesses the physical, emotional, and intellectual skills to live, learn, and work in their own environment. Respite services are provided when deemed necessary by the CAT Team. The Lead Technician works to support the overall goal of stabilization of client crises to reintegrate into the community.
