Adult Targeted Case Manager/Gateway/356, 493/WD

An Adult Targeted Case Manager is responsible for assessing, linking, planning, monitoring and advocating for recipients who have serious mental illnesses to provide for their care while living in the community. Case managers facilitate continuity of treatment for Peace River Centers catchment area recipient's while at the in psychiatric, substance abuse and medical inpatient treatment and/or a state treatment facility and assist the recipients in accessing the service delivery system subsequent to their discharge. The target population served are persons who have been discharged from an inpatient setting, persons who are in need of linking and monitoring to prevent inpatient treatment, persons who lack the resources or ability to provide for their needs without assistance, and people who have been arrested within the recent past.

Qualifications:

Education:

Bachelor's Degree in a Human Services field from an Accredited College or University

Experience:

Experience preferred but not required

Certifications:

Training:

Targeted Case Management Certification Training, completed with supervisor upon hiring

Assessment

Excellent written and verbal communication skills

Highly organized with the ability to work with minimal supervision

Excellent skills and knowledge of Microsoft Office

Transportation:

Must have reliable transportation and a FL driver's license in good standing

Must carry $25,000/$50,000 bodily injury coverage on personal vehicle and provide proof of coverage

Must be able to drive own vehicle and Peace River Center vehicle

Must be 25 years of age and have a clean driving record in order to drive PRC vehicles

Technology Requirement:

As an employee of Peace River Center, if you are not assigned a company-issued phone, you will be required to authorize the use of the Microsoft Authenticator app on your personal mobile device. This is necessary to access our payroll and communication system, Paycor, and Microsoft email as examples. This will ensure secure and efficient communication with the organization.

Work Environment:

Fast paced work environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. The noise level in the work environment is usually moderate. Must understand and accept the possibility of exposure to inside environmental conditions such as communicable diseases and physical/verbal abuse from an out of control patient/client.
