FACT Program Assistant/Bartow, FL/350/DR
This support member of the FACT multidisciplinary team is responsible for organizing, coordinating and monitoring all nonclinical operations of FACT under the direction of the FACT Team Leader; providing receptionist activities including triaging calls, coordinating communication between the team and participants; and secretarial activities to maintain program operation.

Essential Duties/Requirements/Activities:
- ·Comply with all Center policies and procedures except those for which the employee gives notice of the need for reasonable accommodation and for which the Center can make such reasonable accommodation.
- ·Attends all scheduled work hours, meeting, training, and other center functions.
- ·Order and maintain unit supplies, equipment, and furniture; arrange for repair of office furniture and equipment; record mileage logged on program vehicles; and arrange for maintenance of program vehicles.
- ·Record admissions and discharges to provide a source of information for statistical reports.
- ·Scan documents into client electronic health record as needed.
- ·Type letters, memos, and reports as needed.
- ·Prepare letters to request medical records and client authorization and maintain log of requests and records received.
- ·Design and maintain forms to obtain and maintain insurance information regarding all FACT persons, including running monthly reports on which insurance participants have.
- ·Sort, route, and prioritize incoming mail with distribution to appropriate staff.
- ·Greet people at FACT headquarters and answer telephone calls including performing the following:
(1) triage and coordinate communication between the team and persons;
(2) get answers to questions for people, families, community resources, and agencies;
(3) work with callers to relieve urgent situations or to temporarily manage them until other staff are available;
(4) attend to symptomatic behavior (shouting and pacing) of persons who come into the FACT office for appointments with staff or of those who come in without a clear purpose (persons without something to do or who come in because of distress);
(5) decide when and how quickly to refer calls to other staff.
- ·Assess and report clinical behaviors and information (such as poor self-care, medical problems, and increased overt symptoms such as agitation) that are out of the ordinary for that particular client.
- ·Prepare the monthly work schedule.
- ·Collect necessary data and prepare monthly and quarterly FACT reports.
- ·Order and distribute city bus passes monthly to participates, keeping a record of passes issued.
- ·Assist participants with obtaining needed benefits, including Food Stamps and reduced phone service.
- ·Assist medical personnel with obtaining Prior Authorizations for medications from insurance agencies.
- ·Communicate regularly with financial guardians and protective payees of persons to coordinate individual patient budgets between the FACT program and guardians or payees.
- ·Manage and operate the system to document all state contingency fund and cash transactions with receipts signed by persons upon receiving cash.
- ·Communicate with landlords regarding client rental agreements and payment of rent.
- ·Complete FACT reports as required by program contract.
Qualifications:
Education:
Must have a HS diploma/GED
Experience:
Must have at least two years of office experience

Complete all PRC required trainings
Knowledge and Skills:
Strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
Ability to work independently with minimal supervision.
Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for person's rights and personal preferences in treatment.
Good written and verbal communication skills.
Knowledge of medical records management and personnel policies and procedures.
Working knowledge of Excel.
Strong organizational skills.
Ability to establish relationships to get information necessary for the program to function within the larger system.
Ability to work as part of a team.

Technology Requirement:

Work Environment
The work environment is fast-paced and consists of exposure to physical conditions typical of a normal office environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. Most of the job is performed while sitting, although the work may require occasional standing or walking and/or the lifting and carrying of small objects. A small amount of travel may be required for meetings etc. to another PRC location.
