LPN/Primary Care Clinic/2199/CC

LPN position requires an employee be able to react to change productively. Perform Nursing Assessments- including triage of appointments as needed. Administer injectable medication and blood draws as ordered by the prescriber, along with other diagnostic clinic testing. Record vital signs, Weight, BMI, BP. Facilitate prior authorizations. Must complete documentation required for clinic visits. Assist patients by taking information from them over the phone and conveying this to and from the prescriber. .The target population are persons who are uninsured or underinsured and qualify for services in the Low Income Pool grant.

  • An employee in this position must be able to react to change productively, take vital signs and health history- measure vital signs, BMI, phlebotomy, office duties, injections, and other nursing duties and assess the physical condition of patients as needed.
  • Administer injectable medication, blood draws and assessments as required by the prescriber
  • Triage of appointments
  • Complete documentation required for clinic visits.
  • Medical Department Liaison with other departments in the Center and the community as needed for continuity of care
  • Be responsible for communications regarding patient needs including but not limited to over-rides for prescribed medications and referrals to other programs.

Qualifications:

Education:

Valid Florida Nursing License, current and in good standing (LPN).

Completing of continuing Medical Education as required for licensure in the state of Florida: Biennial 24 contact hours: 1 hour Domestic Violence and 1 hour AIDS

Experience:

Prefer 1 year experience working with adults and children in an out-patient or in-patient medical setting. "Bilingual (Spanish) preferred, not required".

Certifications:

Graduated from an accredited school of Nursing (LPN)

CPR

Training:

The employee must meet all requirements for Credentialing within the agency

20 hours professional enhancement training annually including all PRC required trainings

Attends all scheduled work hours, meeting, training, and other center functions

Knowledge and Skills:

Good written and verbal skills

Excellent interpersonal communication skills

Technology Requirement:

As an employee of Peace River Center, if you are not assigned a company-issued phone, you will be required to authorize the use of the Microsoft Authenticator app on your personal mobile device. This is necessary to access our payroll and communication system, Paycor, and Microsoft email as examples. This will ensure secure and efficient communication with the organization.

Work Environment:

Fast paced work environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. The noise level in the work environment is usually moderate. Must understand and accept the possibility of exposure to inside environmental conditions such as communicable diseases and physical/verbal abuse from an out of control patient/client.
