CAT/OnTrack LPN/Lakeland, FL/1796/TF

This member is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, and providing management and administration of medication in conjunction with the CAT/OnTrack psychiatrist/APRN.

Essential Duties/Requirements/Activities:

  • Document client progress to maintain permanent record of client activity according to established policies and procedures
  • Participate in daily staff organizational meetings and treatment planning review meetings on days assigned to work
  • Under direction of CAT/OnTrack psychiatrist/ARNP, evaluate and chart psychotropic medication effectiveness, complications, and side effects; and arrange for required lab work according to protocol.
  • Under direction of CAT/OnTrack psychiatrist/ARNP, organize and manage the system of assisting client with getting medications and integrating medication administration tightly into clients’ individual treatment plans, facilitate medication refills, complete prior authorizations for prescribed medications
  • Educate the client and/or guardian about appropriate medication management
  • Under direction of CAT/OnTrack psychiatrist/ARNP, manage pharmaceuticals and medical supplies
  • Communicate with pharmacies regarding authorizations/refill requests/availability of medications
  • When needed, assist families in setting up medication boxes, organizing medication, and proper disposal of discontinued medication (This may be done in the office or in the home)
  • Complete paperwork required for clients to receive medication at school and communicate with school nursing staff regarding any changes
  • In collaboration with CAT/OnTrack psychiatrist/ARNP, coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment.

Qualifications:

Education:

Graduation from an accredited Nursing ProgramCompletion of continuing medical education as required for licensure in the State of Florida

    Experience:

    Prefer at least one year of experience in an inpatient or outpatient psychiatric setting

      Certifications:

      Active current LPN Florida Nursing license in good standing required

        Training:

        • Employee must meet all requirements for credentialing within Peace River Center
        • Complete all PRC required trainings
        • Complete all Nursing contact hours required for license renewal
        • Attend all PRC clinical and administrative functions deemed appropriate by supervisor
        • Complete Aggression Control Technique (ACT) training

        Knowledge and Skills:

        • Computer skills- Microsoft Office, CMHC and Internet, EHR
        • Ability to work independently with minimal supervision; good organizational skills
        • Good written and verbal skills
        • Ability to respond appropriately in stressful situations while maintaining self-control
        • Ability to communicate appropriately and professionally in stressful situations with clients and/or peers
        • Ability to prioritize work load and multi-tasking when needed
        • Understanding of OSHA standards (Occupational Safety Health Administration) in the workplace and PRC Biomedical waste plan
        • Knowledge of medical terminology, psychotropic medications, and common side effects in work environment
        • Knowledge of effective verbal intervention and aggression control techniques

        Safety Equipment

        • Life Safety Equipment (fire extinguisher)

        Transportation:

        • Must have reliable transportation and a Florida Driver’s License
        • Must have a clean driving record and be able to pass a physical
        • Must meet PRC requirements to drive company vehicles
        • Company cars are provided and will be used as available. At times, a company car may not be available at the time you may need to go into the community to provide services. During those times, you must use your personal vehicle and will be awarded travel reimbursement according to Peace River Center policies. However, CAT/OnTrack participants are never to be transported in personal vehicles. The company vehicles will be reserved for participant transportation.
        • Must provide and maintain proof of personal vehicle coverage $25,000 - $50,000 bodily injury liability

        Machines, Tool and Equipment Used:

        • Computer, multi-line telephone, fax, copier, hole punch
        • Glucometer, B/P equipment, stethoscope

        Technology Requirement:

        As an employee of Peace River Center, if you are not assigned a company-issued phone, you will be required to authorize the use of the Microsoft Authenticator app on your personal mobile device. This is necessary to access our payroll and communication system, Paycor, and Microsoft email as examples. This will ensure secure and efficient communication with the organization.

        Work Environment

        The work environment is fast-paced and consists of exposure to physical conditions typical of a normal office environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. Most of the job is performed while sitting, although the work may require occasional standing or walking and/or the lifting and carrying of small objects. A small amount of travel may be required for meetings etc. to another PRC location.
